Job Description

GuildCare, Lighthouse Guild’s innovative community-based Adult Day Health Care Program providing services to adults who are blind or visually impaired and have chronic health care needs is seeking an Occupational Therapist for its Manhattan location. Responsibilities: Plan, develop, implement and evaluate occupational therapy service and standards. Assure that therapy personnel and registrants follow established policies and procedures at all times. Review occupational therapy policies as requested and participate in making recommendations for changes. Maintain a working relationship with registrant’s community physician. Review requests for occupational therapy and physician‘s orders. Interview and evaluate registrant to determine type of treatment and schedule therapy. Explain occupational therapy procedures to registrant, prepare necessary equipment, instruct and supervise registrant during required therapy. Complete vision rehabilitation assessments for each registrant in the areas of communication skills, orientation and mobility, and activities of daily living. Develop and implement vison rehabilitation treatment plans for those registrants needing vision rehabilitation training at the GuildCare site. Teach communication skills, home management skills, personal care skills and orientation and mobility following the established GuildCare Core Curriculum for Vision Rehabilitation. Serves as a resource person with respect to services, equipment, supplies and materials for the visually impaired. Participates in team development of daily living, educational, cultural and recreational program for registrants. Participate in development and implementation of registrant care plans, registrant assessments and discharge planning, etc., as necessary. Comply with established safety precautions at all times. Complete necessary forms, reports, etc., and submit them to the Program Director as required. Maintain treatment records, registrant files and progress notes as required per GuildCare policy. Recommend to the Program Director the equipment and supply needs of the department. Assure that the therapy area is maintained in a clean and safe manner for registrant comfort and convenience by assuring that necessary equipment and supplies are maintained to perform such duties/services. Assure that all therapist notes are informative and descriptive of the care provided and of the registrant‘s response to the care. Physical demands: Standing for long periods, walking, lifting/transferring registrants, using lifting equipment when required. Bending, stooping, squatting, twisting, reaching above and below shoulder level. Able to push/pull registrants in wheelchairs. Able to lift and carry equipment and/or supplies 20.Responsible for following the LGI Workforce security policy; will access information systems and ePHI authorized by Clinical Authorization only. 21.Other responsibilities as deemed necessary and appropriate by the Program Director. 22. Responsible for maintaining the confidentiality of registrant protected health information. 23. Observes and implements the Registrants‘ Bill of Rights. Qualifications: A Bachelor‘s or Master‘s degree with New York State license and certification is required. Experience in hospital/clinic setting and needs of visually impaired population, preferred.
