Current Positions Right at Home in Perth South Eastern Suburbs is seeking a motivated, reliable Care Coordinator to join our team. This role involves delivering quality in-home care under various funding sources, liaising with clients, families, staff, and stakeholders to ensure personalized services aligned with client needs and goals. The Care Coordinator will handle assessment, planning, and review of care plans, manage sales inquiries, and develop referral sources. Responsibilities include building strong client and family relationships, assigning staff, monitoring client progress, referring to additional services, and maintaining accurate records. The role requires flexibility, responsiveness, and collaboration to meet the needs of a dynamic environment. Qualifications The ideal candidate will: Build strong client and family connections Assign staff based on qualifications and client preferences Support clients with disabilities and monitor progress Refer clients to appropriate agencies Assess client needs for health and welfare services Respond promptly to inquiries and requests Maintain incident and feedback records Prioritize tasks in a fast-paced setting Support clients in achieving their goals Conduct care assessments and planning Supervise and manage care staff Promote services to medical professionals and referral sources Report to head office Ability to work efficiently in a fast-paced environment is essential.
