Care Coordinator

WISCS.org seeks a compassionate and organized Care Coordinator to join our team, dedicated to enhancing public welfare and social justice. This role involves managing comprehensive healthcare services for clients to promote optimal well-being and integration within the community. The successful candidate will be instrumental in assessing client needs, developing personalized care plans, and coordinating with various healthcare providers and social services to ensure seamless service delivery. Assess the health and social needs of clients through consultations and collaboration with other healthcare professionals. Develop and implement personalized care plans tailored to individual client needs in concert with other health care professionals. Coordinate all aspects of patient care, including scheduling, transportation, and access to medical and psychological resources. Act as the primary point of contact between the clients, their families, healthcare providers, and community resources. Regularly review care plans and adjust strategies as needed to achieve the best outcomes for the client. Maintain detailed documentation of client interactions, services provided, and care plans for compliance and quality assurance purposes. Educate clients and their families about their conditions and the care needed to promote their health and well-being. Ability to effectively communicate with patients, families, and medical professionals. Strong organizational and time-management skills. Proficient in electronic health record (EHR) software and data management tools. Compassionate and empathetic approach to sensitive social and health issues.
