Care Coordinator - Youth CCS

As a Care Coordinator for Youth Comprehensive Community Services (CCS) at WISCS.org, you will be instrumental in developing and implementing personalized care plans for youth in the community. This role emphasizes enhancing the well-being of young individuals through coordinated efforts across various services and supports, aligning with our mission to foster a just and equitable society. The Care Coordinator will act as a vital link between young people, their families, service providers, and other relevant stakeholders to ensure the delivery of holistic, culturally competent, and effective care. Conduct comprehensive needs assessments to formulate tailored care plans. Collaborate with healthcare providers, social workers, educators, and other professionals to coordinate services. Regularly review and update care plans to reflect the changing needs of youth. Serve as an advocate for the needs and rights of young individuals. Provide crisis intervention services as needed. Maintain accurate documentation and records in accordance with legal and professional standards. Engage families in the care process, providing them with guidance and resources to support their children's development. Monitor progress and outcomes, adjusting strategies and interventions as necessary.
