Nurse Practitioner

Upward Health is seeking a compassionate and dedicated Nurse Practitioner to join our team. We specialize in providing exceptional, personalized medical care to complex patients directly in their homes. As a Nurse Practitioner with Upward Health, you will play a critical role in extending thoughtful, patient-centered healthcare to those with chronic and acute conditions, ensuring they receive the highest quality of care in the comfort of their own environment. Conduct comprehensive assessments of patients to determine their medical conditions, needs, and preferences. Develop and implement care plans that cater to individual health requirements. Prescribe medication and recommend lifestyle adjustments to better manage conditions, avoiding unnecessary hospitalizations. Monitor and evaluate patient progress and modify treatment plans as needed. Collaborate with a multidisciplinary team to coordinate healthcare services and patient advocacy. Educate patients and their families on their conditions and home health care procedures. Maintain complete and accurate health care records and reports. Ensure compliance with federal, state, and local regulations and ethical standards.
