Care Specialist

As a Care Specialist at Upward Health, you will be a pivotal part of a dedicated team focused on providing top-notch, home-based medical care to complex patients. This role involves working closely with patients, primarily in their homes, to deliver personalized health services according to tailored care plans. You will collaborate with multidisciplinary team members to ensure holistic and continuous care, aimed at improving patient outcomes and overall quality of life. Conduct regular home visits to assess patient health status and needs. Implement personalized care plans developed in conjunction with healthcare providers. Monitor patient health status and progress, and adjust care plans as needed. Provide guidance on medication management, dietary plans, and exercise regimens. Educate patients and their families on health maintenance and disease prevention. Coordinate with doctors, nurses, and other healthcare professionals to deliver integrated care. Document patient care activities and outcomes, maintaining accurate and up-to-date records. Assist patients with daily needs, such as personal hygiene and mobility, when necessary. Strong interpersonal and communication skills. Ability to work independently in a home setting. Proficient in using medical software and patient care technologies. In-depth knowledge of disease management and home health regulations. Cultural competency to deal with diverse patient populations.
