Care Specialist

As a Care Specialist at Upward Health, you will be a cornerstone in providing tailored medical care to complex patients within the comfort of their own homes. This role demands a compassionate approach and a commitment to delivering patient-centric health solutions. You will work collaboratively with a multidisciplinary team to assess patient needs, implement care plans, and ensure the highest quality of service. Conduct initial patient assessments and regular follow-ups to gauge progress and health status. Develop, implement, and revise individualized care plans based on patient evaluations. Coordinate with physicians, nurses, and other healthcare professionals to ensure comprehensive delivery of services. Administer medications and treatments as prescribed and monitor reactions or side effects. Educate patients and their families on health maintenance and disease prevention. Maintain patient records with accuracy, completeness, and compliance with regulatory requirements. Provide emotional and psychological support to patients and their families.
