Position Details: Job Title: Home Health RN Location: Shelton, CT Schedule: Full-time, 40 hours per week (Monday-Friday, 8:00 AM - 5:00 PM) On-Call: One weekend per month Patient Population: Adult and Geriatric Productivity: 25-32 units per week Mileage Reimbursement: $0.48 per mile (visit to visit) Benefits: Medical, Dental, Vision, 401(k) after 90 days Pay: Negotiable based on territory budget and candidate experience Key Responsibilities: Patient Care: Conduct comprehensive assessments and provide skilled nursing care to patients in their homes, ensuring care plans are executed with precision. OASIS Documentation: Complete OASIS (Outcome and Assessment Information Set) documentation accurately to ensure proper patient care planning and compliance with regulatory standards. Homecare Homebase: Utilize Homecare Homebase (HCHB) software to document patient visits, manage care plans, and communicate with the healthcare team. Coordination: Collaborate with physicians, physical therapists, and other healthcare professionals to deliver coordinated, patient-centered care. On-Call Support: Provide weekend on-call support on a rotational basis, addressing urgent patient needs. Patient Education: Educate patients and their families on disease management, medication administration, and lifestyle modifications to promote optimal health. Requirements: Active Connecticut Registered Nurse (RN)License Current Basic Life Support (BLS) certification from the American Heart Association or American Red Cross Minimum of 1 year of experience as an RN in a home health setting Proficiency in OASIS and Homecare Homebase (HCHB) software required Reliable transportation and a valid drivers license Ability to complete Joint Commission onboarding standards Location Details: Shelton, CT, is a welcoming community located in Fairfield County along the scenic Housatonic River.
